Rome’s Oldest Fully Operating Historic Pharmacy
Rome is a city where history surrounds you at every turn. Ancient ruins, Renaissance churches, and Baroque fountains tell the story of centuries past. Yet sometimes the most remarkable pieces of history are hidden inside everyday places. One such place is the Antica Farmacia Reale, located in the historic center of Rome near the Spanish Steps. What makes this pharmacy extraordinary is not simply its age, although it is certainly old. What makes it truly remarkable is that it is still operating today as a fully functioning pharmacy. Many historic pharmacies in Rome and elsewhere in Italy have been preserved as museums or converted into perfume shops or boutiques, but Antica Farmacia Reale continues to do exactly what it was originally created to do: serve the public as a pharmacy.

I recently visited the pharmacy to record a podcast interview with Giulio Tassoni, the owner of Antica Farmacia Reale, and the experience felt like stepping into a living piece of Rome’s past. The interview took place in a quiet office beneath the shop floor, where thick stone ceilings immediately reveal just how old the building is. Upstairs, the pharmacy itself still operates today beneath a beautiful frescoed ceiling. Customers come in to fill prescriptions and purchase medicines, just as people have done for centuries. Giulio prefers to remain behind the scenes and did not appear on camera, but he generously shared the remarkable story of the pharmacy and the documents that prove its long history.
Antica Farmacia Reale: A Pharmacy with Documented History
The Antica Farmacia Reale can document its history back to 1672, which is already extraordinary for a commercial establishment that is still operating today. What makes this claim especially compelling is that Giulio possesses a series of original documents tracing the pharmacy’s ownership from that time forward. These records show the pharmacy passing from one owner to another across centuries, creating a rare and continuous historical record. Giulio has spent years studying these documents and researching the pharmacy’s past, and although he continues to search for earlier records, the documentation he already possesses clearly demonstrates the longevity of this remarkable establishment.

One of the most fascinating documents is a handwritten deed of sale from 1697. The script is beautiful and detailed, written in the elegant handwriting typical of the period. The document records the transfer of ownership of the pharmacy and includes a meticulous inventory of everything inside it. Each item is listed along with its weight, because the value of a pharmacy at that time depended heavily on the quantity of ingredients it contained. Herbs, minerals, and other medicinal substances were carefully weighed and recorded as part of the sale. Holding such a document is an extraordinary experience because it connects the present-day pharmacy directly to its earliest days and provides tangible proof of its long history.
Giulio continues to investigate the pharmacy’s origins and hopes to discover documentation that predates the 1672 records. Finding such documents is not easy because it requires extensive research in historic archives and libraries, and often the assistance of specialists who understand where these records might be hidden. Even so, the documents already preserved provide strong evidence that the pharmacy has existed continuously for centuries.
The Meaning Behind the Name Antica Farmacia Reale

The name Farmacia Reale, which translates as “Royal Pharmacy,” dates back to the eighteenth century when the pharmacy was authorized to provide medicines to the royal household. During that period only a small number of pharmacies in Rome had the privilege of supplying the royal court, and this special status gave the pharmacy its name. The legacy of that relationship can still be seen today in several historical elements preserved inside the shop.
Among the most striking features are the ceramic apothecary jars that line the pharmacy shelves. Many of these jars bear the royal crest, a reminder of the period when the pharmacy served the royal household. These jars were not merely decorative objects but essential tools of the pharmacist’s trade. They once held the medicinal ingredients used to prepare remedies by hand, and the names of the substances they contained are still written on them. In this way the jars serve both as historical artifacts and as evidence of the kind of work pharmacists performed centuries ago.
A Rare Historic Pharmacy That Still Operates
Rome has several historic pharmacies, but many of them no longer function as pharmacies today. Some have become perfume shops, others operate primarily as historic displays, and a few preserve their interiors but no longer dispense medicines. Antica Farmacia Reale is different because it continues to operate as a licensed pharmacy. Customers can walk in with prescriptions, purchase medicines, and receive pharmaceutical advice exactly as they would in any modern pharmacy.
Maintaining a historic pharmacy in the modern world is not easy. Modern pharmacies require technology, lighting, storage space, and compliance with strict regulations. At the same time the historic interior must be preserved, which limits the kinds of modifications that can be made. Giulio explained that this means the pharmacy staff often has to work harder than a typical pharmacy would because they must adapt modern needs to a historic environment without damaging its original features. Even simple changes, such as installing lighting or adding storage, must be done carefully to protect the historic structure.
For Giulio, preserving this environment is not just a business decision but a personal commitment. He believes that places like this help people understand Rome’s history and the traditions that shaped the city. Keeping the pharmacy intact allows visitors and customers to experience a living connection to the past.

A Family Stewardship Beginning in 1948
Although the pharmacy itself dates back centuries, the Tassoni family has owned it since 1948, when Giulio’s grandfather purchased the business shortly after the end of World War II. At that time Rome and much of Europe were rebuilding after the devastation of the war, and Giulio’s grandfather saw the opportunity to continue the life of this historic establishment. He initially bought the pharmacy together with a partner, but within a couple of years the partner left the business and Giulio’s grandfather became the sole owner.
Since then the pharmacy has remained in the Tassoni family. Giulio himself began working there when he was seventeen years old and gradually developed a deep connection to the space and its history. Over the years he has come to know every corner of the pharmacy and has dedicated himself to preserving it. His brother chose a different career and became a lawyer, so Giulio is now the primary caretaker of the business. His daughter is still young, and he does not yet know whether the next generation will continue the tradition. For now his focus is simply on maintaining the pharmacy and protecting its historic character.
A Laboratory Tradition That Continues
One of the most fascinating aspects of the pharmacy is that it still maintains a laboratory for preparing medicines. In earlier centuries pharmacists did not simply sell medicines manufactured elsewhere. Instead they prepared remedies themselves according to prescriptions written by doctors. Although modern pharmaceutical manufacturing has largely replaced this practice, some pharmacies still prepare specialized medications when a doctor prescribes something that is not commercially available.
Antica Farmacia Reale continues this tradition. If a doctor prescribes a customized formulation, the pharmacy can prepare it in its laboratory. This work requires special authorization and strict compliance with modern regulations, but it allows the pharmacy to continue a practice that dates back hundreds of years. In this way the pharmacy remains connected to the historical role pharmacists once played in medical care.

Historic Tools and Apothecary Equipment at the Antica Farmacia Reale
The pharmacy also preserves a collection of historic tools used to prepare medicines. These include mortars and pestles, pill molds, measuring devices, and other instruments that were once essential to pharmaceutical work. Some of these pieces are displayed inside the pharmacy, while larger items are stored safely at Giulio’s home because space in the shop is limited.
These tools provide a vivid picture of how pharmacists once worked. Medicines were carefully mixed, ground, measured, and shaped by hand, and the process required considerable knowledge and precision. Seeing these instruments alongside modern pharmaceutical products highlights how much the profession has evolved while still maintaining connections to its past.
Reviving an Old Pharmacy Tradition with Amaro
Another interesting aspect of the pharmacy’s work is its connection to amaro, the herbal liqueur traditionally served after meals in Italy. Historically many herbal liqueurs were developed in pharmacies and monasteries as medicinal tonics intended to aid digestion. Giulio recently revived that tradition by creating an amaro inspired by historical formulas.
The ingredients are listed on the bottle, but the exact proportions remain secret because the recipe belongs to the pharmacy. Like many traditional amari, the drink combines herbs and botanicals to create the distinctive bitter flavor Italians enjoy after dinner. Although originally conceived as a digestive remedy, it is also simply a pleasant drink to enjoy at the end of a meal.
A Modern Addition: Artisan Perfumes
Giulio has also introduced a line of artisan perfumes. While perfume is not traditionally sold in Italian pharmacies, he saw an opportunity to expand the pharmacy’s offerings in a way that still connects to its heritage. Working with a specialized laboratory, he developed nine fragrances, each with its own story and inspiration. The perfumes are produced artisanally, and even the packaging is handcrafted.
Visitors from outside Italy have shown particular interest in these fragrances, which combine craftsmanship with the historic atmosphere of the pharmacy. In this way the pharmacy continues to evolve while remaining rooted in its long tradition.
A Living Piece of Rome’s History
What makes Antica Farmacia Reale so remarkable is not simply its age but the fact that it still functions today exactly as it was intended. Customers come in to fill prescriptions, pharmacists prepare specialized medicines, and historic jars, tools, and documents remain connected to everyday work rather than being locked away in a museum. The pharmacy stands as a rare example of a historic establishment that continues to serve its original purpose.
For Giulio, preserving this continuity is essential because it allows people to understand the history of Rome and the traditions that shaped the city. Standing inside the pharmacy, surrounded by centuries old documents and tools, it is impossible not to feel the passage of time. In a city already famous for its monuments and history, Antica Farmacia Reale offers something even rarer: a place where the past is still actively at work in the present.
